Massage Client Forms - for all new Prenatal Massage clients of Massage By Brie are necessary, so that I can obtain your current pregnancy related health history. It will save time, if you print the Massage Intake Form in advance, fill out, and have it ready for me on your 1st appointment.

Once you arrive for your first appointment, I will review the form with you, and we can discuss any issues that you are having, or any massage goals or questions that you have for the session.
